Online Registration Terms of Use

 

To use the online registration, you must be the legal cardholder, or someone who is legally authorized to use the card and at least 18 years of age.  Any unauthorized or fraudulent use of a credit card in conjunction with this registration site is strictly forbidden and any information resulting from such actions will be turned over to the credit card company and the authorities.

 

Registration Policy

• Payment is due in full, with a completed Registration Form, to register for the program.

• Oke Hockey reserves the right to change the time of a program.

• No refunds or credits will be made for absences missed by the participant.

Participation Waiver

 

The Participant and the Participant’s Parent/Legal Guardian hereby acknowledge that:

 

1. The participant has voluntarily applied to participate in the Oke Hockey summer program.

2. The Participant and the Participant’s Parent or Legal Guardian understand that the act of playing hockey involves risk of injury to the Participant and other people, which includes but is not limited to death, permanent or temporary paralysis, disability, physical or mental damage or other injury, as well as damage to the Participants equipment and personal property.

3. Should any injury occur to the Participant, permission is granted to the staff of Oke Hockey to assess the Participant and seek necessary medical treatment including, but not limited to administering prescribed medication, transportation of Participant to a medical facility, facilitating an X-ray or other emergent treatment recommended by a Physician.

4. Oke Hockey (including any and all staff and volunteers) assumes no liability for loss, damage, or any kind of injury sustained by the Participant or the Participant’s property while participating in camp activities.

5. The Participant and the Participant’s Parent or Legal Guardian therefore assume all risk associated by participating.

6. By signing the release of liability and participating in the camp, the Participant and the Participant’s Parent or Legal Guardian hereby fully release and discharge Oke Hockey from any and all claims, demands, damages, rights of action or causes of action present or future, resulting from or arising out of the Participants participation.

7. The Participant and the Participant’s Parent or Legal Guardian agree to indemnify Oke Hockey and hold it harmless from any and all liability attributable to any personal injury, or loss or damage to property related to the Participant’s participation.

8. The Participant and the Participant’s Parent or Legal Guardian acknowledge any photos, videotaped footage, or audio recording taken of participants are for Oke Hockey purposes only.  Images will only be used for promotions and/or educational use of Oke Hockey.

 

Privacy Policy

 

Oke Hockey collects information for intended use, and manages accordingly. Oke Hockey adheres to the ten principles for organizations under the Personal Information Protection and Electronic Documents Act (PIPEDA).
